Privacy Policy

Effective date: August 1, 2025

Kadar Orthodontics (“Kadar,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains how we collect, use, disclose, and safeguard information when you visit gokadar.com, contact us, or use our online services (including virtual consults and appointment forms). This Policy covers website and general personal information. For health information (PHI) created or received in the course of your care, please see our HIPAA Notice of Privacy Practices, which controls in the event of any conflict. Kadar Orthodontics - Sewell, NJ

1) Information we collect

  • You provide: name, contact details, messages, scheduling details, insurance information you choose to submit, photos you upload (e.g., for virtual visits), and preferences (e.g., communication choices).

  • Collected automatically: device/browser data, IP address, site pages viewed, and interactions via cookies or similar technologies.

  • From third parties: service providers supporting our website, analytics, forms, messaging, and patient engagement tools (see “How we share information”).

2) How we use information

We use information to:

  • Provide our services (scheduling, consultations, virtual visits, responding to inquiries).

  • Operate, secure, and improve our website and user experience.

  • Communicate with you (including service- or appointment-related texts or emails if you opt in).

  • Comply with law, protect our rights and users, and prevent abuse, fraud, or security incidents.

PHI: When information constitutes Protected Health Information (PHI), we use and disclose it only as permitted by HIPAA and our HIPAA Notice of Privacy Practices (for example, for treatment, payment, or health care operations). 3) Online tracking, cookies, and analytics

We (and our vendors) may use cookies and similar technologies to remember preferences, measure site usage, and improve performance. You can control cookies through your browser settings; blocking some cookies may affect site features. Your browser may send “Do Not Track” signals—our site does not currently respond to these signals. We do not allow third-party behavioral ad targeting on pages where you submit health information, and we configure tools used on patient-facing pages to avoid collecting PHI or we avoid using such tools on those pages altogether, consistent with HIPAA guidance.

4) How we share information

We share information:

  • With service providers that host our site, process forms, provide analytics, communications, security, and related services—only for their work on our behalf and under confidentiality commitments (and Business Associate Agreements where required by HIPAA).

  • As required by law or to protect rights (for example, to comply with valid legal process or to protect our patients, staff, and property).

  • In corporate events (e.g., reorganization) as permitted by law, with appropriate protections.

We do not sell personal information. We also do not “share” personal information for cross-context behavioral advertising as defined by some state privacy laws. If this ever changes, we will update this Policy and provide applicable opt-out mechanisms.

5) SMS/texts and emails

If you opt in, we may send appointment reminders, updates, or service messages via SMS/text or email. Message and data rates may apply. You can opt out of texts by replying STOP and out of non-essential emails via unsubscribe links. (Transactional messages about your care may still be sent as allowed by law.)

6) Your choices

  • Access/Update: You may update contact details you have provided by contacting us.

  • HIPAA rights (PHI): Patients have rights to access, request amendments, request restrictions, receive an accounting of disclosures, choose confidential communications, and obtain a paper copy of our HIPAA Notice. See our HIPAA Notice of Privacy Practices for how to exercise these rights. Kadar Orthodontics - Sewell, NJ

  • State privacy rights: Depending on where you live, you may have additional rights regarding certain website-collected personal information (e.g., to know, access, correct, delete, or opt-out of specific processing). We will honor valid requests as required by applicable law. State privacy laws continue to evolve; we periodically review our practices for compliance. IAPP

For information about how we use and disclose Protected Health Information and your HIPAA rights, please review our HIPAA Notice of Privacy Practices (available on our website and in our office).

ADDENDUM TO NOTICE OF PRIVACY PRACTICES: 2026 REGULATORY UPDATES
Effective Date: February 16, 2026

This Addendum amends the Notice of Privacy Practices to include mandatory information required by federal law regarding Substance Use Disorder (SUD) records and new notice requirements.

  1. Protections for Substance Use Disorder Records. If we receive or maintain your records from a federally assisted substance use disorder treatment program (a “Part 2 Program”), these records are subject to stricter federal confidentiality rules (42 CFR Part 2) than other health information. We will only use or disclose these records for treatment, payment, and healthcare operations if you have provided a single, general consent, or as otherwise permitted by federal law.

  2. Prohibition on Use in Legal Proceedings. Records protected by 42 CFR Part 2, or any testimony describing such records, shall not be used or disclosed in any civil, criminal, administrative, or legislative proceedings against you by any federal, state, or local authority without your specific written consent or a court order meeting federal requirements.

  3. Fundraising Rights. If your SUD records are used for fundraising purposes, you have a clear and conspicuous right to opt-out of receiving any such future communications.

  4. Notice of Potential Redisclosure. Any health information disclosed by this office pursuant to our Notice of Privacy Practices or this Addendum may be subject to redisclosure by the recipient and may no longer be protected by federal or state privacy laws.

  5. Accounting of Disclosures. You have the right to request an accounting of disclosures made by us of your SUD records through an electronic health record for the purposes of treatment, payment, and healthcare operations.
